Job Summary:

Lifelong’s Aging and Disability Services (ADS) program provides support and Medicaid case management to enable clients to reside in the residence of their choice and avoid living in a skilled nursing facility. The ADS Case Manager provides person-centered services and supports for vulnerable adults through promoting the efficient use of resources, emphasizing the least restrictive interventions, and building on the strengths of clients and their family or active support networks. This is a full-time, non-exempt, union, regular position.
