Information and Assistance

Community Living Connections (CLC) provides older adults, adults with disabilities and their caregivers with information and assistance about navigating long-term care systems, community resources and medical benefits. For assistance beyond our scope, we provide referrals to other CLC providers and community partners.

This service is funded by the City of Seattle Human Services Department.  


For Older Adults and Adults with Disabilities 

  • Information about available resources and benefits including:

    • Options for long-term services and supports

    • State and federal benefits, including Medicaid

    • Care coordination

    • Transportation

  • Assistance obtaining needed resources and information

  • Warm handoffs to other CLC providers


For Caregivers

  • Referral to caregiver services and supports

  • Information about community resources and benefits

  • Assistance obtaining needed resources and information

  • Warm handoffs to other CLC providers
