Assessment & Consultation with a Registered Dietitian Nutritionist
Nutrition counseling, often called Medical Nutritional Therapy, includes an assessment of a client’s nutritional health and individual dietary needs. Our knowledgeable Registered Dietitian Nutritionists (RDNs) work one-on-one with clients, looking at things such as food intolerances, allergies, accessibility to healthy food, diet, medical test results, and mental health to support them in achieving them.

ELIGIBILITY
We provide nutrition counseling at no cost for clients enrolled in Lifelong services living with:
Pre-diabetes and diabetes
High cholesterol
High blood pressure
Congestive heart failure
Kidney disease
Gastrointestinal (GI) issues such as IBS, Crohn’s, and ulcerative colitis, constipation, acid reflux, and other GI symptoms
Unintentional weight loss or weight gain
Chewing and swallowing difficulties
Food allergies and sensitivities
Other conditions which can be supported with nutrition
